- ▸ Bonafide resident of Himachal Pradesh
- ▸ Medal in Olympic, Asian, CWG, or National Games
Benefits
- ✓ Parshuram Award: ₹2 Lakh
- ✓ Olympic Gold: ₹3 Crore; Silver: ₹2 Crore; Bronze: ₹1 Crore
- ✓ Asian Games Gold: ₹30 Lakh

- ▪ Proof of age (birth certificate / school certificate)
- ▪ Domicile / residence proof
- ▪ Recent passport-size photograph
- ▪ Sports achievement certificates (state / national)
- ▪ Bank account details (passbook copy)
