- ▸ Medal winner in recognized National/International events
- ▸ Annual income below the threshold defined by the state (indigent status)

- ▪ Recent passport-size photograph
- ▪ Sports achievement certificates (state / national)
- ▪ Bank account details (passbook copy)
- ▪ Coach / academy recommendation letter (if applicable)
- ▪ Medical fitness certificate
